The CDSMP workplace program is one hour, twice a week for 6 weeks. It meets online on Tuesday and Thursday afternoons from 12:00pm to 1:00pm EST, from Tuesday January 28 through Thursday March 6, 2025.
Due to funding regulations, priority will be given to those living in Southeastern Ontario.
The Chronic Disease workplace program provides information and skills to those living with one or more chronic conditions at work, helping them better manage their condition and achieve their best health and wellness. Techniques to deal with symptoms of chronic conditions, such as fatigue, pain, sleeplessness, shortness of breath, stress, and emotional problems such as depression, anger, fear, and frustration.
This FREE workshop is for adults of all ages living with chronic diseases such as diabetes, heart disease, lung disease, cancer, and depression, among others. These workshops are also open to those at risk of developing a chronic disease, as well as family members and caregivers.
The workshops are led by two trained leaders who are trained volunteers with chronic conditions themselves.
Participants report improvement in their lifestyle and general health, as well as improved symptom management and better communication with their doctors. You will receive a copy of the book Living a Healthy Life with Chronic Conditions, as well as the audio file Relaxation for Mind and Body.
This workshop is an evidence-based self-management program originally developed at Stanford University and currently supported by the Self-Management Resource Center. Physicians, registered dietitians, occupational and physical therapists, and other health professionals have reviewed all workshop materials.
