FREE workshop that helps to actively achieve best health & wellness while living with a chronic condition.

This is a 6-week class, once a week on Mondays, October 17 to November 21, 2022 from 1:30 to 4:00pm EST.

There is also a 15-30 minute Zoom orientation session, exact date and time TBD (session is obligatory if you are not familiar with Zoom, and highly recommended even if you are). If you are not able to make the session and require help with Zoom, an alternate one-on-one orientation session can be scheduled.

Zoom links and workshop details will be sent to you closer to the date from a "@kchc.ca" email address and not from EventBrite.

Because of the way that we are funded, priority will be given to those living in Southeastern Ontario. If spots remain about a week before the start date, then we will open the session to those living elsewhere in Ontario.

This workshop will be offered via Zoom Meeting. You will need to download the software, but it is free and you do not need an account. You must also be comfortable sharing your video. If you are not comfortable with doing so, please sign up for one of our sessions of an At-Home Kit with Support Calls instead (if none are listed, please feel free to contact us to be placed on a notification list for future ones - [email protected]).

The Living Well with Chronic Disease Workshop provides information and skills to those living with one or more chronic conditions, helping them better manage their condition and achieve their best health and wellness.

This FREE workshop is for adults of all ages living with one or more chronic diseases such as diabetes, heart disease, lung disease, cancer, and depression, among others. These workshops are also open to those at risk of developing a chronic disease, as well as family members and caregivers. Participants meet in groups of 5 to 16 individuals for 2.5 hours, once a week for 6 weeks, and will also receive a free copy of the book Living a Healthy Life with Chronic Conditions and the audio CD Relaxation for Mind and Body. The workshops are led by two trained leaders who are usually volunteers with chronic conditions themselves.

Participants report improvement in their lifestyle and general health, as well as improved symptom management and better communication with their doctors. They also spend less time in the hospital compared to those who have not taken the workshop.

This workshop is an evidence-based self-management program originally developed at Stanford University and currently supported by the Self-Management Resource Center.
